The Account Application Form is used to establish business accounts on the Web site. Submitting criminal history checks through the web site with a billable account can only be done after an application is verified and an account created. Please make sure all mandatory fields are completed (mandatory fields are in bold.) A business account for this site will be created after information is verified. The Open Records Unit will then contact you with login information.
